Lower Blepharoplasty vs Lower Eyelid Filler

Filler (Restylane, Juvederm, etc) can be an effective way to improve the appearance of eye aging. Hyaluronic Acid, the main ingredient in filler, is a common in office treatment that can address a variety of areas of volume loss. These areas can include the folds between the cheeks and lips (nasolabial Folds), the lips themselves, temporal Hollows, etc.

However, depending on the anatomy and goals of patients, often filler alone will not adequately address the appearance of the lower eyelids. If the issue is simply fat volume loss and a look of deflation, then filler is likely a good first line treatment for improvement

But, if the main concern is bags under the eyes (which are fat pockets), filler will not correct this. Additionally, if the tear trough ligament is causing a harsh transition from the lid to the cheek region, then filler may help some but will not fully correct this.

The analysis of the lower lids is very nuanced, but the above can help introduce treatment options to patients. In addition, lasers and other non-surgical treatments like microneedling and microdermabrasion can help optimize a surgical result or even be used as stand-alone treatments depending on the anatomy and goals of the patient. Postoperatively, filler can also be a very powerful to get an even better result when surgery may not be able to fully correct volume loss in some patients.
